Regarding the UTV accident, based on HS Supt. and Safety Officer experience, my professional opinion is the driver was either inexperienced or an aerial-delivered FF. And very likely also inattention, speed, no seat belts, and no safety helmets all as causal factors!
I agree with Mr Stubblefield, "Were there consequences? All too often, there are none; no one learns and those who know what they did wrong, are simply scolded by a superior."
Their usual venues, e.g. the Facilitated Learning Analysis and / or Learning Review and / or Coordinated Response Protocol (CRaP) using the Orwellian "Group Interviews" never determine fault or lay blame. In addition, they almost never give details on the weather so that we'd at least be able to research the RAWS site and / or fire weather station(s) on our own.
So then, what are the alleged "Lessons Learned" garnered from the Reviewers and / or Investigators - yet almost never revealed? There rarely are any. So then, that leaves it up to all of us - the individual and collective readers - to determine our own subjective conclusions.

Was the driver of the UTV going too fast? if so, was he/she reprimanded or demoted in classification?
